Office Policies
When you visit us at Panorama Oral Surgery, your oral health, dental conditions and well being are our foremost priority.
For your convenience, Panorama Oral Surgery's office policies are as follows:
Payment Options
Payment for services is due at the end of each appointment. In some cases, payment is made ahead of treatment, as when sedation is administered, our policy is to have you return home as soon as possible after safe discharge.
We accept the following methods of payment:
- Debit
- Visa
- Mastercard
Insurance
We will prepare and submit all necessary insurance forms on your behalf. The majority of these are submitted electronically, allowing for your insurance company to reimburse you as soon as possible. Depending on your plan, this may occur within 10 days of submission.
Please keep in mind that dental insurance is an agreement between each patient and their specific insurance provider. While we are pleased to assist in the submission of information and forms, dental insurance limits on some policies can be lower than the cost of some procedures. Additionally, some dental policies exclude certain types of procedures.
Each patient is responsible for all dental charges incurred in the course of their care. We are happy to preauthorize your treatment plan and send it to your insurer who will then inform you of your coverage.
We will work with your insurance provider to ensure that your receive the maximum amount of coverage for your dental procedure.
Cancellation Policy
Appointments are scheduled specifically for you! Our team contacts each of our patients 1 to 2 days before an appointment with a courtesy reminder.
It is, however, the patient’s ultimate responsibility to keep each scheduled appointment. We require a minimum of 24 hour’s notice to change and/or cancel appointments. If an appointment is missed, a missed appointment fee may apply.
Privacy
Panorama Oral Surgery complies with all privacy laws regarding the collection, use and storage of patient health information. These include the Personal Health Information Act (PHIA) and the Freedom of Information and Protection of Privacy Act (FIPPA).
In all cases, personal health information gathered will consist only of that which is required to safely deliver oral surgery care either in the office or in hospital.
This information will be safeguarded on the office premises under direct supervision of front desk staff during business hours, and in a locked and alarm-protected space at night.
Requests to view and correct personal health information will be fulfilled immediately upon request. If patients request a copy of their chart, it will be provided in a timely fashion. In some cases, there may be an administrative fee for this service.
All inquiries regarding the privacy policy or compliance with PHIA or FIPPA may be directed to Dr. Blair Dalgliesh. » Contact
